Student Registration - PreK & Younger & SpecEd

PLEASE PROVIDE THE FOLLOWING INFORMATION AND FORMS FOR REGISTRATION:

Pre-K and younger, and those being evaluated for special education services:

• Proof of residency. Refer to the Proof of Residency Requirements Chart for acceptable proof
• Proof of Age
• Complete the Residency Questionnaire (Form A) - one per family
• Complete the Registration Packet (Form C) - additional pages available for each student being registered
• Proof of immunizations (For those requiring evaluations ONLY)
