Library card needed:

  • Get a Library Card - Every person living in the Farmingdale School District is entitled to a free library card. Simply provide us with 2 forms of ID that include your name and current address.
  • Examples of acceptable forms of ID for proof of residency are:
    • car insurance
    • car registration, checkbook with name and address
    • license
    • and utility bill.
  • Children not in Kindergarten will need the signature of a library card parent/guardian and a birth certificate.
  • Children in Gr. K-8 will need the signature of a parent/guardian and their Student ID.
