Transcript Requests

Former students should provide the information listed below to request transcripts. This request can be sent via email, you can call our office at 516-434-5290, or you can fill out our Official Transcript Request Form to request a transcript. Please include all requested information when you contact us.

Provide the following:

Year you graduated 
Your name at graduation
Where you would like your transcript sent - Please provide the complete address and include a contact person if applicable.
Note whether you need an official transcript, an unofficial transcript, or both.

Please note that official transcripts will be sent in sealed envelopes and are void if the seal is broken.
